The Hospice Social Worker is a professional who coordinates and directs support services to hospice patients and caregivers in their home environment (private residence, nursing home facility, assisted living facility, hospital, etc.) based on individual needs. The Social Worker facilitates the provision of supportive services to assist to meet the needs of the patient and caregivers, with a holistic approach, keeping with the patient and caregiver's beliefs and goals of care. The Social Worker coordinates with the Hospice Interdisciplinary Team, patient, caregivers, providers, Medical Director, referral sources, and community sources to work toward plan of care goals for each individual patient. The Social Worker documents in the patient's medical records in a timely manner according to agency requirements. The Social Worker provides patient care based on provider's orders, departmental policies and procedures, regulatory guidelines and best practice. The Social Worker attends and actively participates in team meetings.
We provide service to patients in the following counties: Kerr, Kimble, Kendall, Gillespie, Bandera, Real, Medina and Edwards County that are within a 50-mile radius from our office location in Kerrville, TX We offer mileage reimbursement at the current IRS rate. We utilize the HomeCareHomeBase electronic medical record system. Peterson Hospice is a department of Peterson Health, which allows additional opportunities since we are integrated with a hospital system. Peterson Hospice has received the Kerrville Reader's Choice Award for Best Hospice from 2017-2022. Peterson Hospice has a 5 star rating on the Medicare, Care Compare website (https://www.medicare.gov/care-compare). Required Education:
Master's degree in social work
Required Technical Skills:
Generally accepted standards of practice in respective discipline. Professional and efficient verbal and written communication skills. Basic computer skills. Ability to be open, sensitive, flexible and ecumenical. Must have demonstrated experience in working with patients and caregivers dealing with life threatening illness and end of life. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Required Licensure/ Registration/ Certification:
Current Texas state license in appropriate discipline; Current Texas state driver's license; Current auto insurance. Must have reliable transportation.
Required Experience:
Two years' experience in a healthcare setting in respective discipline.
